How to Write a Nursing Case Study Report in Nepal: Structure and Care Plan

Nursing students in Nepal write case studies throughout PCL, BSc Nursing and BN programmes, and the same weakness appears again and again: pages of textbook disease description followed by a thin care plan. Your instructors already know the pathophysiology. What they are assessing is whether you can apply the nursing process to one real patient.

The standard structure

Compare the book with the patient

The disease review earns marks when you set it against your patient directly: a two-column comparison of textbook signs and symptoms versus those actually present, with a note on which were absent. This one table shows clinical observation rather than copying, and it takes ten minutes.

Write nursing diagnoses correctly

A nursing diagnosis is not a medical diagnosis. Use the standard three-part form: the problem, related to the cause, as evidenced by your assessment data — for example, "Ineffective airway clearance related to retained secretions as evidenced by crackles on auscultation and productive cough." The evidence must come from your own assessment section; if it is not documented earlier in the report, it cannot appear here.

Care plan columns and priorities

The rationale column is the one students skip and instructors read closest. Every intervention needs a reason grounded in physiology or evidence, not "as per doctor's order".

Confidentiality and ethics

Use initials or a pseudonym, never the patient's full name, hospital registration number or identifiable photographs. Obtain verbal consent and record that you did. Nepali nursing programmes take this seriously, and identifiable patient data in a submitted report is a genuine problem, not a formatting slip.
